At STRIDE® Orthotics and Pedorthic Center, we strive to achieve the highest level of patient satisfaction possible. To maximize your time with your clinician, we ask that you become familiar with the following protocols:

Prior to your visit:

Our office staff will complete an intake, check your insurance benefits, inform you of pricing, advise you of any additional paperwork needed from your doctor(s) and/or other providers/agencies, and schedule your appointment. We will also offer you the opportunity to receive a link via email to complete a portion of your registration forms via our Onpatient registration portal.

Your First Visit

Time

Please plan to arrive approximately 15 minutes prior to your appointment to complete paperwork. Plan to be with us for approximately 60 minutes.

Bring

A photo ID, your insurance card(s), your referral/prescription from your doctor (if applicable), any old orthotics/inserts that you may have.

Wear

Comfortable clothing and shoes (wear or bring sneakers if you have them).

Registration Forms

Save time by downloading, printing, and completing your registration forms prior to your arrival and bringing the completed forms with you (if you have not completed your forms electronically).

Insurance

We participate in the following:
Anthem BC/BS-Commercial and Managed Medicare
Aetna-Commercial and Managed Medicare Connecticare-Commercial and Managed Medicare
Traditional Medicare and Connecticut State Medicaid including Husky Plans through the State of Connecticut.
Our benefits verification department will work with you to determine out of network coverage with your plan when applicable.
